Registering as a patient at Billingshurst SurgeryRegistering as a New Patient

We have an open list and welcome requests for registration from patients living in or moving to the practice area. New patients must live within our practice boundary to be able to register with us. Because of the fluid nature of our practice boundary there is a chance that we may not be able to accept you as a patient - we will check eligibility when you come in to register. We will also ask for proof of residence that you live within the surgery boundary.

All patients wishing to join the practice should come into reception and make an appointment for a new patient medical check. Please be aware that no discrimination will be made in regard to race, gender, social class, age, religion, sexual orientation, appearance or on grounds of disability or medical condition. Your medical records can sometimes take a while to follow you to a new practice and so this new patient consultation helps us to get to know your medical needs as soon as possible. If you live within the practice area and wish to register, please complete either your medical card or print off and complete the New Patient Registration Form and return it in person to the surgery with photo id.

Practice Boundary

The proposed changes to Practice Boundaries and the ability to register with a GP of choice, as announced in the press on 17th September 2009, have not been formally adopted and therefore the Practice Boundary remains the same. The proposed changes are suggested to be implemented within 12 months but have not been formalised. We will amend this page as appropriate.

Currently we can accept patients from Billingshurst, Wisborough Green, Shipley, Coolham, Kirdford, New Pound, Barns Green, Slinfold, Itchingfield and some limited areas of Pulborough, Southwater and Broadbridge Heath.

Change of Personal Details

If you change your name, address or telephone number please remember to inform the surgery. This may be done either by visiting the surgery, phoning us or submitting thisĀ Change of Personal Details form (PDF file).

More About Your NHS Number

Each NHS Number is a unique 10-digit number which is printed on your NHS medical card or will from now on be written on any communication you have had from the NHS.
