Registering With Us

We have an open list and welcome requests for registration from patients living in our practice area. New patients must live within our practice boundary to be able to register with us. Due to the fluid nature of our practice boundary there is a chance that we may not be able to accept you as a patient - we will check eligibility when you come in to register. You can use the Practice Boundary Tool below to check yourself if you are unsure.

If you are registering with us please be aware it can take up to 5 working days to register you once your forms have been submitted.  You will receive a text, email or letter to confirm you have been registered. 

We advise all patients considering registration to obtain at least one months supply of medication from their current GP surgery before registering, to ensure there is ample time for records to transfer over. Almost all surgeries in England can now send electronic prescriptions to any pharmacy of your choice so you current surgery should be able to do this even if you are now in our area.

If you would like to register with the practice please complete this online NHS GP registration request. If you are researching for someone who doesn't have internet access or you have any trouble with the online registration tool you can pick up paper copies of our registration forms at front desk.

Your medical records can sometimes take a while to follow you to a new practice, so please bear this in mind if you have appointments in the first few months of being registered at a new practice. Please be aware that no discrimination will be made in regard to race, gender, social class, age, religion, sexual orientation, appearance or on grounds of disability or medical condition.

Mobile Numbers & Email Addresses

We use text messages to send appointment reminders, routine test result information, and to communicate with you directly. If you provided us with your mobile number you will be automatically subscribed to this service and we will assume that this includes your consent to contact you using these details, including text messages and emails. Please note that it is your responsibility to update us with any changes to your contact details. If you do not wish to receive text messages from us please let us know. Similarly, we may contact patients by email for all-patient or other notifications, eg Seasonal Flu Clinic alerts, so please let us know if you do not wish to receive emails from us.

Registering as a Temporary Resident

If you are temporarily staying (more than 24 hours) in our catchment area you can register with the surgery as a ‘short stay’ (up to 14 days) or ‘long stay’ (up to three months) Temporary Resident if you meet the following criteria:

  • You require ‘immediate & necessary’ urgent medical attention
  • You are more than 20 miles away from your usual registered GP surgery
  • You are a child staying locally

We do not accept Temporary Resident registrations for the purpose of requesting repeat medication. Instead, we ask that you contact your own surgery and ask them to send a prescription to a local pharmacy who can dispense for you. Our local pharmacies can accept electronic prescriptions, please contact them to discuss how they can get a prescription from your usual surgery.

Practice Boundary

We have a fairly large practice boundary, currently we can accept patients from Billingshurst, Wisborough Green, Shipley, Coolham, Kirdford, New Pound, Barns Green, Slinfold, Itchingfield and some limited areas of Pulborough, Southwater and Broadbridge Heath.

Please note that we are able to change our boundary to include/exclude areas over time, if we move the boundary leaving you on the outside we will still continue to serve you as a patient. However, please note that if you move home, and your new home is outside the boundary (even if the new home is closer), we will be unable to register you/continue to see you as a patient.

Change of Personal Details

If you change your name, address or telephone number please remember to inform the surgery. This may be done either by visiting the surgery, phoning, submitting this change of personal details form (PDF file) or logging in to Patient Access and updating your personal information (you must be registered to use this online service).

More About Your NHS Number

Each NHS Number is a unique 10-digit number which is printed on your NHS medical card or ,will from now on be written on, any communication you have had from the NHS.

Blood & Organ Donor Registration

When filling out the GMS-1 registration form to join us as a new patient, you will be offered the chance to sign up to the NHS national blood door and organ donor schemes.
